Add Phone Number and Email Address Fields for GP in Deceased Details

In Deceased Details section Under the GP Field, adding phone number and email address fields for the GP. The functionality will include:

• Displaying the phone number and email address for the selected GP.

• This will function similarly to the Funeral Director section, enabling users to access this information without needing to navigate away from the deceased record to the setup page.
