Clinician Cause of Death to be added to Medical Examiner Scrutiny

Multiple hospitals have expressed an interest in having a dedicated area for cause of death (as issued by hospital clinician) to be entered and available within the Medical Examiner scrutiny (in between ‘A’ forms and ‘B’ forms) - as per rough mockup example. The intention is for this data to be completed by MEOs and for this to allow Medical Examiners easy access to the cause of death whilst carrying out their scrutiny. Suggestion is to title the tab ‘CCOD’ (Clinician issued cause of death) The new section will allow entry of Cause of death 1a, 2b, 1c,1d and 2 (alongside comments and remarks free text boxes) . Please feel free to comment on whether this feature would be useful, and if you have suggestions on the title and placement of the new tab.

Adding to the Bereavement pathway

The data below is already in the ME Form A1 in the drop box where it says ‘No communication with the NR’, but is it possible this question and drop box can be replicated  inbetween the two fields below on the Bereavement Pathway – MCCD Status and can it populate from the ME Form A1? The reason is when all paperwork is completed and we have to call relatives to confirm it would be easy to miss the info on the A1 form, but it is needed there as that is when initial calls are made and the box populated by the MEOs Bereaved declined interaction with medical examiner office No response from bereaved. No informant/next of kin Other ( Unable to add the field location)
