Add a Warning message when opening a deceased record.

Add a Yes/No slider for "Enable Warning" under the "Action Required Yes/No" section.

If the "Enable Warning" slider is set to "Yes," it should display a warning message with the content listed in the "Action Required Notes" every time someone opens a deceased record.
